Do I need to turn in a notorized medical release form for my players?
No, the medical release on the mvysa forms page is there for your convenience if you go to a tournament that requires it or you want to have it. mvysa does not require that form. mvysa does require the parent/guardian to sign both the medical and liability part of the USYSA Player Membership form.
